All students, including international students, must provide 1) proof of residency, 2) immunization records, 3) birth certificate, 4) social security card, 5) most recent report card, 6) most recent transcript, 7) disciplinary records and 8) special program information if applicable (special education, 504, gifted and talented, etc.).
Please contact the campus registrar at your current campus to discuss availability at our other campus locations. If your job or a change of address takes you to another area of the metroplex, you can easily transition to another Life School campus. If you enroll at any Life elementary school, you can easily transition into Life Oak Cliff or Life Waxahachie for middle or high school.
The following count as proof of residence: 1) Current property tax bill with parent/guardian's name and property address; 2) Current rental or lease agreement with the parent/guardian's name, student name, and address, as well as manager or owner's name and telephone number; 3) Documents related to the purchase of the residence with the parent/guardian's name and property address; 4) Mail dated within 60 days before the application date from the following sources: Social Security Administration; A Texas State government agency; Utility companies; Credit card bill; Financial institutions (including checking or savings); Insurance companies; State and Federal Revenue documents; or Pay check information. You can contact your campus Registrar to provide proof of residence.
